What is hospital setting?

When you have like 2 or 3 pillows behind your back on a soft bed (Tempurpedic Bed Preferred)..

Guy 1: When I watch TV in my room I lean against my wall.

Guy 2: Wow.. When I watch TV in my room I have a Hospital Setting.
